VALUES OF THE SERUMS OXIDATIVE STRESS INDICATORS IN THE AGE-RELATED CATARACT PATIENTS

Actual hypothesis related to development of the age-related cataract focus the photo oxidative stress as a key process of cataract genesis. It considers that damages in the lens cells, caused by sunlight and oxygen within photodynamic action of the photo oxidative stress, make essence of the cataract genesis process. Although the cataractogenes is primary a local process that flows in a local compartment, oxidative stress is often a systemic event. It relates particularly to the older age, i.e. to the ageing process itself. Even if special regulatory mechanisms of the eye provide appropriate supply of antioxidative elements on the local level, fulfilling of the eye and it's lens with antioxidative elements depends on their systemic availability and distribution. By investigation of concentrations of malondialdehyde, vitamin C, vitamin E, glutathione and albumin, and activity of cathtalase and peroxidase in serums of patients with age-related cataract, as well as with comparison of the same elements values in serums of the same age and sex distribution group of people without cataract, we found noticeable differences of the oxidative stress intensity in serums within those two investigated groups.

THE EFFECT OF ASPIRIN AND TICLOPIDINE ON LIVER FUNCTION TEST IN RATS

The purpose of this study was to evaluate the effect of aspirin and ticlopidine, administered alone and in combination, on liver function parameters. The experiment was conducted on white laboratory rats, type Wistar. Thirty-two rats were divided in four groups and they recived one of the following treatments for 4 days: group I, control, saline (1 ml/kg, i.p.); group II, aspirin (50 mg/kg/day i.p.); group III, ticlopidine (125 mg/kg/day i.p.) and group IV, aspirin+ticlopidine combination (50 mg/kg/day+125 mg/kg/day i.p.). After the treatment the animals were anaesthetised with ether and blood for further analyses was taken by cardiopunction. The total cholesterol serum level was significantly increased only in ticlopidine group in comparison to control (p<0.01).Also, the serum activity of alkaline phosphatase and total bilirubin concentration were significantly elevated only in ticlopidine treated group (p<0.001). Serum AST and ALT activities were not significantly elevated in all treating groups. On basis of the obtained results it can be noticed that the values of liver function parameters are greater in group treated with ticlopidine than in group treated with ticlopidine and aspirin combination.

HISTOCHEMICAL LOCALISATION AND ACTIVITY OF DPP IV IN ANTIGEN STIMULATED TONSILLA

Dipeptidyl peptidase IV is ecto-enzyme which is present on the cell surface of large number of cells and particularly of T-lymphocytes. It takes part in immune reactions and regulation of immune response, while antigen stimulation of cells increases percentage of expression of DPPIV on their cell surfaces. Since the immune response is different for chronic tonsillar disease of tonsilla palatine, idiopathic tonsillar hyperplasia ITH and recurrent tonsillitis RT, and considering the fact that DPPIV takes part in activation of T and B-lymphocytes stimulated by antigen, we presume that different activities of DPPIVexist in tonsillis and blood of persons diseased of different types of chronic tonsillitis, ITH and RT. The main goals of research are determination of activities of DPPIV on lymphocytes of tonsillas and serum of persons diseased of ITH and RT as well as histochemical and immunohistochemical determination of DPP IV localisation on lymphocytes of examed tonsillas. Two groups of patients are included: nineteen persons with ITH and thirteen persons with RT, while the control group was contained of thirty nine persons with diagnosis systematic lupus erythematosus (SLE) and fiftteen health persons. The research was done on 32 tonsillas created by tonsillectomia of persons with ITH and RT and 86 blood samples of all 86 persons. The results of research show histochemical localisation of enzyme activity of DPPIVin T-dependent zones of tonsilla and it is originating from activated T-lymphocytes. The enzyme activity of DPP IV on tonsillar lymphocytesis is higher in case of IHT compared with RT. By this research we conclude that deifferent activity of DPPIV on tonsillar lymphocytes for ITH and RTpresumes the difference of immuno response of tonsillas during these tonsillar diseases.

AEROBIC PARAMETERS OF ELITE SPORTSMAN

Parameters of anaerobic capacity were investigated in the group of 90 young subjects classed into diferent groups depent on their physical fitness. The group consisted of 30 wrestlers, 30 rowers and 30 students. Basic antropometric parameters were taken for each person and than Wingate anaerobic test was administrated. The aim of this investigation was to measure anaerobic parameters and determine weather there was a difference among sportsman and non sportsman. All parameters of Wingate test (anaerobic power, explosive power, anaerobic capacity) were significantly higher in the group of sportsman. (p0.05) Also, the highes values od anaerobic parameters were detected in the group of wrestlers, since this is typicaly anaerobic sport. The lower values were measured in the group of non sportsman, as expected.

QUALITY OF LIFE IN CORONARY PATIENTS AFTER CORONARY AND VALVULAR SURGERY

There are different reports regarding the characteristics of examined quality of life (QOL) in patients with combined coronary and valvular surgery. Objective was to compare the quality of life changes after isolated coronary and associated coronary and valvular surgery. From February to May 2002, we prospectively studied 243 consecutive patients, who underwent elective CABG. The Nottingham Health Profile Questionnaire part I (NHPpart I) was used as a model for QOL determination. NHPpart I contains 38 subjective statements divided into six sections: physical mobility (PM), social isolation (SI), emotional reaction (ER), energy (En), pain and sleep. The scores ranged from zero to 100, by adding the item weight, determined by Thurstone method of paired compares, to every positive answer. We distributed the questionnaire to all patients before and six months after CABG. Two hundred twenty six patients filled in the postoperative questionnaire. An isolated coronary heart disease was present in 218 patients (89.71%), and associated coronary and valvular disease in 25 patients (10.29%). Preoperatively, there is no difference in QOLamong the examined groups. After operation, quality of life was improved in 84.65% (171/202) of patients using coronary surgery and in 79.17% (19/24) of patients using combined (coronary and valvular) surgery. Quality of life was statistically improved in the first group of patients in all sections, and in the second group of patients in 4 sections (PM, En, pain and sleep) (p<0.05). There is no significant correlation between the quality of life changes after operation and the type of surgical procedure. By univariate and multivariate logistic regression, combined procedure distinguished as the independent predictor of QOL worsening in the ER section (p=0.012, OR=3.62, 95% CI 1.32-9.92). Six months after CABG quality of life was significant better in most of patients in both studied groups. Associated coronary and valvular surgical intervention distinguished as the independent predictor of quality of life worsening in the ER section.

DEYING OF ELASTIC TISSUES WITHOUT DIFFERENTIATION: NEW, QUICKLY METHOD

Our methodological procedure established successful technique of the direct dyeing of elastic fibers and lamellas in elastic tissues. Experiments were carried out on a human autopsy material. Blocks rich in elastic fibers were fixed by 10% buffered neutral formaline and cut in the thickness of 5m. Deparafining of slides is done by xylol treatment (2x15min). Additional denaturation is achieved by combined solution, which contains 1.2g of picric acid in 30% solution of glacial acetic acid. By chloramine B application as blocking agent in form of 1% DMSO solution the dyes affinity for eviromental tissue is decreased. Tissues prepared in this way are treated with 0.5% solution of acidic sulfonic color Evans Blue (C.I.23860) in ratio 2:1. Lamellas are dark violet with light violet periphery. Obtained results speak in favor of high applicability of this method in light microscopy. In our opinion, this method can be recommended as one of methods for identification of elastic tissues.

EFFECTS OF THE ACE INHIBITORS ON THE LEFT VENTRICULAR FUNCTION IN THE ESSENTIAL HYPERTENSION

In essential hypertension, hemodynamic changes in systolic and diastolic function acompany left ventricul hypertrophy ussualy. Both of these cardiac function can be detected with echocardiography and were measured during this study. 23 patients with moderate essential hypertension were treated with cilazapril plus small doze of diuretic (hydrochlorothiazide) once daily for 120 days. On the begining we found diastolic disfunction, but after the treatment there was a significant improvement in isovolumic relaxation time, early diastolic filling pattern and normalized left ventricular early/ atrial filling ratio. Cilazapril + diuretic combination in a dose once daily controlled blood pressure satisfactory, too. Parameters of systolic function were in the normal levels and there were no significant changes between the two determinations. Thickness of the interventricular septum and posteroinferior wall did not change significantly after 16 weeks of therapy, probably because of too short time of investigation.

THE ROLE OF BACTERIAL VAGINOSIS IN ETIOLOGY OF PRETERM DELIVERY

Ahigher concentrations of microorganisms in the vagina can lead to preterm delivery. Bacterial vaginosis is a well known risk factor of preterm delivery. Aim of this study was to estimate the presence of bacterial vaginosis as a potential cause of preterm delivery relative to the patients with term delivery, as well as to define the type of present bacteria. The results of our investigation showed more frequent presence of bacterial infection of the vagina in the patients with preterm delivery. The following bacteria have been cultured in the group of patients with preterm delivery: Streptocossus agalctiae, Enterococcus, Staphylococcus sp. and Escherichia coli. In prevention of preterm delivery, we recommend a routine screening of pregnant women (such as obtaining smears to bacterial infection) as well as administration of the therapy in case of positive findings, particularly in those presenting a risk for the development of bacterial vaginosis and preterm delivery.

WHETHER EXISTED CARDIOCEREBRAL SYNDROME?

Besides big improvement of diagnostic, therapeutic and preventive procedures vascular disease generally and vascular disease of brain remain of biggest importance in total morbidity and mortality. This examination included patients in Urgent Surgery Center “Simonida” Gracanica. In article is represent relationship between brain and heart dysfunctions and it is obvious that cerebral disturbances can cause heart disturbances and also it can be in opposite direction. Well, based on this is very important to make fast and correct orientation: CARDIOCEREBRAL or CEREBROCARDIALSYNDROME. In diagnostic procedure and therapeutic treatment is necessary cooperation between cardiologist and neurologist because in most of these cases we have cardiocerebral or cerebrocardial syndrome. Better treatment patients with brain stroke claim teamwork different specialist.

OBESITY, DIABETES AND CORONARY HEART DISEASES

The clinical and metabolic anomalies observed in patients with type 2 diabetes are associated with high risk of cardiovascular disease (particulary coronary heart disease), witch is responsible for 75% of all death in diabetic patients. Diabetic people have cardiovascular risk factors comparable to those of nondiabetics who have had myocardial infarction or stroke. To determine main risk factors for coronary heart desease (CHD) and differentes among diabetics we analyzed 86 patients.Patients in this study attended in Institute of endocrynology,diabetes and matabolic disorders in Belgrade. The diagnosis was based on typical clinical manifestation: high levels of glucose (higher than 6,1mmol/l), high cholesterol and tryglicerides, hypertension and overweight. In the control group we have had 20 adult, healty people without metabolic disorders. We found that diabetics with coronary heart disease have had the higest level of glucose (13,2mmol/l), cholesterol (6,96mmol/l),tryglicerides (2,84mmol/l). Arterial hypertension in the group of diabetic patientes with CHD have had 72,7 %, or 40 patients of 55 patients of whole group. Type 2 diabetes and hypertension share certain risk factors such as overweight, visceral obesity,and possibly insulin resistence. This study strenthgenes the hypothesis that cholesterol and the arterial hypertension have central role in develpoment of CHD, but in non-insulin depended diabetics they associated with metabolic disorders witch play an important role in increasing the risk of coronary heart disease.

EVALUATION OF USAGE ANTIINFECTIVE MEDICAMENTS FOR SYSTEMATIC APLICATION ON THE BASIS OF DEFINED DAILY DOSE

Defined daily dose (DDD) is arbitrary chosen technical unit for measuring medicament usage and it is not a recommended therapeutic dose. It is in fact a statistical parameter for monitoring medicaments usage, and it is related to the dose which is usually prescribed for the indication that the medicament was registered for. The aim of this work is to perform a detailed analysis of the extent and structure of the region of South Backa. Antiinfective medicaments in systematic use, according to the obtained data, participate with only 7.96 DDD/1000 inhabitants. The most applied pharmacotherapeutic subgroup with 2.99 DDD/1000 inhabitants is comprised of cephalexine and cephachlor.

THE MOST FREQUENTLY CARDIAC RHYTHM DISORDERS AT THE SICK OF DECOMPENSATED CHRONIC PULMONARY HEART

Chronic pulmonary heart (CPH) stands for presence of hypertrophy and/or dilatation of right heart ventricle caused by pulmonary hypertension as a consequence of diseases that are damaging lungs function or structure. By chronic pulmonary heart are not considered those hypertrophies of right ventricle which are consequences of left cardiopaths or congenital heart anomalies with skips. During the CPH evolution three stages were formed: a) uncomplicated pneumopathology, b) compensated chronic pulmonary heart and c) decompensated chronic pulmonary heart. The disorders of cardiac rhythm are often at decompensated CPH, which is proved by this scientific work as well. 40 sick people diseased of decompensated CPH are treated in purpose to establish the presence and the characteristics of cardiac arrhythmia at these patients. It is established that the major percentage of frequency is made by sinus tachycardia, atrial extrasystols, and ventricular extrasystols. Of 40 examinees 65% had sinus tachycardia, 37,5% atrial extrasystols, 30% ventricular extrasystols and 20% combined atrial and ventricular extrasystols. This proves us that there is a significant percentage of cardiac rhythm disorder frequency at the sick people of decompensated chronic pulmonary heart.

GENESIS, PATHOPHYSIOLOGY AND TREATMENT OF PREMATURE LABOUR

Premature labour is provoked by noumerous causes that represent a combination of socioeconomic, maternal and fetal factors, that usually act together. The introduction of of tocolytic agents in obstetric practice has not appeared to substantially change the incidence of preterm delivery. This problem is still one of the unsolved ones in perinatal medicine. The aim of the study was to present how structural and functional anomalies of placental physiology can be connected with this disarrangement of pregnancy.In our study of 327 pregnancies controled in our unit in 41 (13,26%) symtoms of premature labour were present: contractions, vaginal bleeding, abrevation and dilatation of cervical ostium. With adequate treatment, in form of combination of tocolytics, spasmolytics, hormones, nonsteroid antiinflammatorz drugs, antibiotics and atimycotics we have posponed delivery to term in 34 (82,92%) of cases. The use of appropriate medications, that blocks pahtophysiological mechanisms responsible for preterm labour in most cases we can enable prolongation of gestation and term delivery.

IMPORTANCE OF CYTOKINES IN REGULATION OF SPECIFIC IMMUNE RESPONSE

In regulation of specific immune responses the most important role play interleukin-2, IL-4, IL-5, IL-13, inter feron- (INF- ), transforming growth factor- (TGF- ) and lymphotoxin (LT). These signal molecules are produced mainly by T-lymphocytes after recognition of foreign antigens by specific receptors (TCR) placed on plasmalema. Some of mentioned cytokines stimulate proliferation and differentiation of various lymphocyte populations in the activation phase of T cell-de pendent immune responses, while the others activate and regulate the function of specialized effector cells, such as mono nuclear phagocytes, eosinophils, and neutrophils, to eliminate antigens in the effector phase of immune responses. In con trast to most of cytokines which have stimulating action on an initiation and course of humoral and cell-mediated immune responses, TGF- has an inhibitory effect on the activation and proliferation of T-lymphocytes and the other leukocyts

DIAGNOSTIC AND TREATMENT OF OSTEOPOROSIS

Osteoporosis is a condition chracterized by a reduced amount of bone which leads to diminihed physical strength of the skeleton and an increased susceptibility to fractures. There are many causes of osteoporosis but the most common by fair is due to the decrese in the amount of bone which occurs after the menopause, the so called postmenopausal osteoporosis. There is no evidence that postmenopausal bone loss itself causes any symptoms and ensuing osteoporosis has therefore been called «the silent epidemic».

POSTMENOPAUSAL WOMEN SEXUALITY

Women sexuality is a complex phenomenon encompassing biological, psychological, emotional and social aspects. The menopausal changes have undoubtedly a significant influence on sexuality. There is a widespread opinion that older women show a fading interest and desire for sexual relations. However, despite of all unavoidable changes brought about by menopause and elder age, the sphere of sexuality remains still very important. The paper reviews the changes occurring in the sphere of sexuality in the older age, influence of menopausal hormaonal changes, psycho-social factors and general health state in these changes, as well as of the possibilities of treating sexual dysfunctions in the menopausal period.

PRIMARY HYPOTHYREOSIS AND ITS CONSEQUENCES ON CARDIOVASCULAR SYSTEM

The thyroid gland produces two major active thyroid hormones, thyroxin (T4) and triiodothyronine (T3). The production of these hormones within the thyroid is regulated by thyreotropin (thyroid-stimulating-hormone- TSH). Iodine deficiency is the most common cause of goiter and borderline hypothireoidsm word-widw. In non iodine deficient areas, however autoimmunity is the most common cause of hypothyroidism. Destructive therapy by surgery or radioiodine for thyrotoxicosis acounts for approximately one third of all cases excessive iodine (e. g. as e result of chronic ingestion of proprietary cough medicines) may also cause hi pothyreoidsm. Primary hypothireoidsm due to failure much more common than failure secondary to pituitary dysfunction, or tertiary hypothireoidsm due to hypothalamic disease. The symptom of hypothireoidsm are non-specific and may be attributed by bath patient and doctor to ageing the onset usually being insidious. Hypothireoidsm is treated with replacement T4 therapy. The author describe a 55 year-old women with diagnosis of Primary hypothireoidism. The disease had a rarely course with disorders this metabolism lipids and complications here cardiovascular system.

DYSPLASIO EPIPHYSIALIS MULTIPLEX - CASE REPPORT

Dysplasio epiphysialis is congenital disorder of ossification and development of epiphysis that lead to deformation of epiphyses. This disorder of is not so frequent (1). Etiology is unknown. It appears in both sexes in the same percent, sporadically, but in the several members of one family also (2). It appears in two forms, polyarticular and oligoarticular. Clinical feature characterize low growing (150-160 cm) with short clumsy fingers, incrassate joints and limited mobility. Especially characteristic of this disorder is discrepancy between low degree of subjective complaints and marked radiological changes. (1). Laboratory findings most often are not specific. Metabolism of calcium and phosphorus is not changed (2). Radiological is characteristic irregular aspect of joint flanges and articulating surfaces and narrowing of the joint spaces, cartilage sclerosis, cystic changes in joints and, not so often periarticular chondromatosis. Vertebral joints if geared manifest platispondilia and ragged of vertebral joints surfaces (2). Conclusion: Dysplasio epiphysialis multiplex is characterized with specific clinical and radiological feature and appearance in jung ages; it is often substitute with primary generalized osteoarthrosis or rheumatoid arthritis (our case). From those reasons it is very important to recognize the specific nature of this disease for the therapeutic and prognostic purpose.

PUSTULOSIS SUBCORNEALIS

Pustulosis subcornealis is a chronic recurrent disorder with primarly sterile pustular lesion, initially bean-sized. The initial distribution is symmetrical and disseminated. The pustules is affected the trunk and the proximal extremities. A rare condition, affecting women. We want to present a woman with a characteristic subcorneal pustular lesions on the clinical findings and histpatology examination.

PHLEGMONA PERIURETHRALIS

We presented you six patients suffering from phlegmona periurethralis. It is mens' disease and, in the greatest degree, it appears at stenosis and urethra trauma. The commonest case of this disease are patients suffering from diabetes and those who carry catheter all the time. The appliance of a large doses of antibiotics and early incision with drainage can help patients to be cured. Urine derivation is very important and it must be like minimal cystostomy

RARE INJURY OF BOTH TESTICLES CAUSED BY FIREARMS

The injuries of genital organs are commonest today regarding to the fact that there are different kinds of firearms and contact mines, although testicles are well protected in thigh area. In cases where we have maximum testicles injuries the conservative surgical treatment should be applied, considering the fact that testicles have great power of "restitucio ad integrum".

CAUSES AND CONSEQUENCES OF SOCIALE DIFFERENCE FAMILY PLANING IN KOSMET

In well organized societies, accept of material and technical progress, planning of the family and existing of human resources is of great importance.
