THE INTERACTIONS OF BIOELEMENTS WITH INCREASED INTAKE OF COPPER

Copper is an essential element, a constituent of all living cells and an integral component of many enzymes. The increase in pollution of the environment, and various metabolic diseases, imbalances biological processes and homeostatic regulated content of bioelements in the body. For the purpose of determining changes in the contents of bioelements with increased intake of copper, the paper followed by interaction before and after ingestion of copper. The results showed that copper in excess causes disruption of physiological interactions of bioelements in tissues and significantly increases the content of iron and magnesium. The nature and relationships of these interactions are important, because in terms of exposure to metals, there are changes in their mutual relations, with some of these antagonistic relationships may be utilized for further study in order to protect them from toxicity.

OCULAR MANIFESTATIONS IN PATIENTS WITH RHEUMATOID ARTHRITIS

Rheumatoid arthritis is a chronic multisistemic disease of an unknown cause characterised by persistens inflammatory synovitis which results in destruction cartilage and bone erosions. RA is a systemic disease with various out of joint manifestations of different clinical importance, among which rheumatoid knots, rheumatoid vasculitis, pleuropulmonal and ophthalmological manifestations are the most prominent ones. The most frequent changes of the eye in patients with RA are keratoconjunctivitis sicca (KCS), episcleritis and scleritis and ulcerous keratitis. Out of 70 examined patients, 52 (74.3%) are female, and 18 (25.7%) of male sex (ratio 4:1), average age 56.47. In 18 (25.71%) RA diseased patients eye changes found are as folows: secondary Sjögren syndrom, that is keratoconjunctvitis sicca in 18.57%, only in female patients, episcleritis nodosa and scleritis in 4.29%; 2.86% in male and 1.43% female patients and ulcerous keratitis in 2.86% RA diseased patients.

UTILIZATION OF ANTIBIOTICS FOR SYSTEMIC USE AT THE SURGICAL CLINIC OF THE CHC - PRISTINA IN GRACANICA

In our work we analyzed utilisation of antibiotics for systemic use at the Surgical Clinic of the Clinical Hospital Centre - Pristina in Gracanica in 2007. and 2008. Data on antibiotic utilization were delivered on the basis of drug dispensing records from the Central Hospital Pharmacy during the two-years period. Antibiotics were distributed inside the ATC/DDD system in accordance with WHO guidelines. Quantitative antibiotics' utilisation was expressed as number of defined daily doses per 100 patient-days (DDDs/100 PD). Overall use of antibiotics in 2007 was 123.37 DDDs/100 PD, and 125.09 DDDs /100 PD in 2008. Three most utilized antibiotics in 2007. were cefuroxime 24.26 DDDs/100 PD (19.66 % of total antibiotics' use), ceftriaxone 16.65 DDDs/100 PD (13.49 %), and cephalexin 15.78 DDDs/100 PD (12.79 %). In 2008. the most utilized were ceftriaxone 23.23 DDDs/100 PD (18.57 %), cefuroxime 22.53 DDDs/100 PD (18.01 %), and co-trimoxazole 19,55 DDDs/100 PD (15.63 %). Qualitative and quantitative part of the consumed antibiotics was mainly in accordance with the results of similar researches in our country.

AN ULTRASOUND DETERMINED FREQUENCY AND VULNERABILITY OF PLAQUES ON CAROTID ARTERIES IN PATIENTS WITH DIABETES MELITUS TYPE 2

Even 1000 years BC the famous Persian physician Aviccena describet the first time gangrene diabetes mellitus as a complication. Basic pathohistological substrate vascular complication is atherosclerotic plaqueand therefore is an adequate treatment opf patients with vascular disease is conditioned by timely and precise diagnosis. The introduction of ultrasound technology in clinical practice enabled Edler and Hertz when the shou moves infarction 1954th,a then and Yoshida (1956.) and Sotomura (1959.), Frenklin (1961.) who are using ultrasound display of blood flow in the courts of man. The aim of our study was to deterimne the frequency of occurrences of atherosclerotic plaque in the carotid arteries and ultrasound examination of differences in the variability of certain plaques between patients with diabetes mellitus type 2 and healthy subjects. The examination was attended by a total of 82 people divided into two group. In first group they are 51 persons suffering from diabetes mellitus type 2 in to second control, the number of healthy respondents was 31. Among our respondents, a significantly higher incidence and plaque vulnerability was ina group of patients with diabetes mellitus type 2.

METABOLIC REGULATION OF DIABETES AND FUNGAL SKIN INFECTIONS

In diabetics fungal skin infections are reported to be related to greatly disturbed metabolism. In order to get better insight into the association between diabetes mellitus and fungal skin infections, we started our study with the aim to assess the effect of metabolic regulation on the incidence of fungal skin infections is diabetics. The obtained results of our study show that in patient with metabolically poorly regulated primary disease i.e. diabetes the incidence of the fungal skin infections is highest (36.0%), its incidence is lower in patients with metabolically satisfactorily regulated diabetes (29.5%) and lowest in those with well regulated diabetes (18.8%), witch is statistically significant difference (p=0.045).

ENDOMETRIAL HYPERPLASIA AND POLYPS

Endometrial hyperplasia appears like a result of continues, not mortified stimulation of endometrial by estrogenically hormones. Endometrial becomes nub which histological mirrors in endometrial gland multiplication, in smaller amount even endometrial strom. Endometrial polyp represents focal overmuch growth of gland stromas endometrial and its considered a focal endometrial hyperplasia. Therefore we will examine them at the same time. Medical faculty of Prishtina - Kosovska Mitrovica did an analysis of biopsy and operational material in pathology institute in the period from 2005 to 2008 out of 3696 histological proved cases, its recorded that 210 cases have endometrial hyperplasia and endometrial polyp, which is 5,68%. According histological type the most often seen is endometrii hyperplasia simplex who is spotted in 127 cases (60,48% - D= 0,195; p<0,001), endometrii hyperplasia complex in 19 cases (9,09%), cases where endometrial hyper-plasia and endometrial polyps are united are spotted in 14 cases (6,67%), while endometrii polypus in 50 cases (23,81%). By analysis of all types of endometrial hyperplasia and endometrial polyps we came to conclusion that all of these changes are mostly made at the age of 41-50 (60,48%) compared to other ages. (D=0,267; p<0,001). In between all patients the youngest was 22 years old (endometrii hyperplasia simplex) and the oldest 78 years old (endometrii polypus). On the basics of studied values, regression's analysis it's been confirmed that the numbers of cases with endometrial hyperplasia and endometrial polyps are growing for 8,6 every year, which points at bigger and bigger problem in the whole pathology.

THE RELATIONSHIP BETWEEN EFFICIENCY TOPIRAMATE IN CONTROL OF EPILEPTIC SEIZURES AND THE APPAERANCE OF COGNITIVE ADVERSE EVENTS

Topiramate is the new antiepileptic drug, which is related with high efficiency in the reduction seazures, and also whit high prevalention of cognitive adverse events. Purpose of study is to define the relationship between efficiency topiramate in control of epileptic seizure and the apperance of cognitive adverse events. In the prospective stady we followed 40 adult patients with refractory epilepsy, which treatment took place on the Institute of neurology Clinical center of Serbia. The efficacy of treatment was estimated as the ratio of the total number of seizures during the last two months before the introduction of topiramate (on basic antiepileptic therapy) and during the last two months on stable (200mg/day) dose of topiramate. All patients are neuropsychological tested twice. First time before the start of therapy with topiramate, and next time two months after the achieving of stable dose. We used neuropsychological tests for evaluation attention, concentration, visual and verbal memory, speech, executive functions, divergent opinion and visual construction abilities. Patients with good success of topiramate in control of the seizures had worse scores on cognitive measurements than patients in whom topiramate was not demonstrated efficacy in treating epileptic seizures. Our findings indicate that the appearance of cognitive adverse events of topiramate possible in patients with reduction of seizure after the introduction of drug. This suggests that the same mechanism topiramat achieve a positive therapeutic effect and adverse effect on cognition in people with epilepsy.

REGULATORY T LYMPHOCYTES

Regulatory T cells represent unique CD4+CD25+ population of T lymphocytes, which are involved in many immunological events. These cells have very important role in maintaining peripheral tolerance, prevention of autoimmune diseases, in immunological respond to infection, particularly by intracellular microorganisms as well as in a process of tumorogenesiz. There are two populations of regulatory T cells: natural regulatory T cells - nTregs and adaptive, inducible regulatory - iTregs. They differ between each other in accordance to the place of creation, in accordance to the specificity of the T cell receptor, as well as the action mechanism. nTregs and the other T cells arise in thymus, which provides specific inductive environment for maturation and immunological education of progenitor cells to imunocompetent T cells. iTregs arise on periphery from the conventional CD4+ T cells after stimulation by antigen under the different conditions. There is number of subpopulations of iTregs and the most important are Tr1 and Th3 cells. Tr1 cells produce big amounts of interleukin 10 (IL-10), and they are called IL-10-secretory regulatory cells, whereas Th3 cells are necessary for maintenance of peripheral tolerance to mucous surfaces, through their dominant cytokine TGF-â (transforming growth factor-â) and they are most widely spread in the mucosa of the digestive tract. Previous researches have shown that regulatory T cells achieve their suppressor activity via inhibitory cytokines, cytolysis, and induction of metabolic disorders and modulation of maturation or function of dendritic cells.
