Iside effects of salicylates (ACK), corticosteroids and imunomodulators on the gastroduodenal mucosa - medication used in rheumatoid arthritis treatment

The aim is to determine pathological -ulcero-erosive changes in the gastroduodenal mucosa (erosions, ulcers, ulcers, ulcer complications, subepithelial hemorrhage, etc.) resulting from the adverse effects of salicylates /ACK/ corticosteroid, imunoodulatora which was prescribed in the treatment of rheumatoid arthritis (RA). The test was performed on 50 patients diagnosed with RA who were on the two-year continuous therapy. When respondents were done gastroscopy and pathological macroscopic changes resulting from the gastroduodenal mucosa as a result of adverse effects of administered drugs verified. After two years of preparations ACK controlled therapy, corticosteroids, imunomodula- sector on mucosa gastroduodenal erosions were found in 81.5% of patients, gastric ulcer in 33%, duodenal ulcer with 18.5% of patients and bleeding in 44.4%.

NEZELJENA DEJSTVA NSAR NA SLUZNICI GASTRODUODENUMA ORDINIRANIH U TERAPIJI REUMATOIDNOG ARTRITISA

Cilj rada je da utvrdi patoloske promene na sluznici gastroduodenuma (erozije,ulceracije, ulkusi, komplikacije ulkusa,subepitelna krvarenja i dr) nastale kao posledica stetnog dejstva NSAIL (Nesteroidnih antireumatika )lekova koji su ordinirani u terapiji Reumatoidnog artrit-(RA). Ispitivanje je vrseno na 100 bolesnika sa dijagnostikovanim RA koji su bili na kontinuiranoj terapiji NSAIL. Kod polovine bolesnika uradjena je gastroskopija da bi se verifikovale patoloske makromorfoloske promene pre kontrolisanog uzimanja NSAIL Posle 2godine uzimanja kontinuirane terapije NSAIL uradjena je kontrolna gastroskopija a patoloske makromorfoloske-endoskopske promene verifikovale. Posle dvogodisnje kontrolisane terapije NSAIL na sluznici gastroduodenuma erozije su opisane u 66% bolesnika, ulkus zeluca u 18%, duodenalni ulkus kod 14% bolesnika, krvarenja u 25% bolesnika.

Clinical, diagnostic and therapeutic aspects of pulmonary embolism

Pulmonary embolism (PE) and venous thromboembolism are two clinical manifestations of a single entity - venous thromboembolism. PE usually occurs through due thrombi from the lower veins, and in 10-15% of cases are due thrombus from upper veins or right heart failure. Significance is mainly determined by the frequency (1/1000 per year in the adult population; less in younger but significant increase with increasing age), the risk of mortality is important in acute and chronic diseases that are associated with this clinical manifestation. Acute pulmonary embolism is often fatal, with a mean annual mortality of about 30% in untreated. Many deaths occur because of recurrent PE within the first few hours after the initial event. In the nineteenth century, Virchow the mechanism of thromboembolism categorized into three groups: changes in the blood vessel wall, a reduction of blood flow (path) and changes in the composition of the blood (hypercoagulable, prothrombotic). Modern interpretation of the pathogenesis of thromboembolism, which is processed corresponds Virhovljevom concept. In general thromboembolic events can be a result of complications of individual risk factors as increased thrombotic potential.

Gastrointestinal stromal tumors stomach cese

Gastrointestinal stromal tumors (GIST omi) are a heterogeneous group of mesenchymal tumors. Etiological factors that cause GIST ome are not clearly distinguished, characterized by remarkable diversity of the clinical, histopathological and genetic features. Mitotic activity, nuclear pleomorphism, degree of cellularity, nuclear-cytoplasmic ratio, tumor size, mucosal invasion, tumor ulceration and necrosis, are factors that determine the malignancy of these tumors. The most common location of these tumors of the stomach and small intestine, while very rare localization of the colon, esophagus and rectum. Despite the progress that has been made in the diagnosis and further diagnosis of intra - or postoperative. The aim of this report is to contribute to easier recognizing the symptoms, diagnosis and treatment of these tumors. It was a mixed tumor, epitheloid-spindle type cells, immunohistochemical analysis, we have a positive expression of CD 117 and CD 34.

INCIDENCE OF HELICOBACTER PYLORI INFECTIONS AMONG THE PATIENTS WITH ULCER AND NON-ULCER DYSPEPSIA

Helicobacter pylori infection is the largest number of infected people in the world. It is believed that 50 - 60% of world population infected, but with uneven schedule. Helicobacter pylori is the most important cause of duodenal and gastric ulcer. The prevalence of infection in different communities vary depending on geographic, racial and socioeconomic factors, and the last 10-20 years in some areas can be observed a significant reduction in the frequency of infection in both the general population and among patients with peptic ulcer. In our country, except in the territory of Vojvodina, there was no serious investigation of frequency of Helicobacter pylori infection in the general population.
