The influence of microbiologic flora on the clinical course of malignant otitis externa

Necrotizing otitis externa, also known as malignant type of the inflammatory process, represents an infection of the skin of external hearing canal, a fast development of tissue necrosis and infection spreading to the surrounding structures. Basic clinical characteristics are: strong earache, leaking from ear, granulation findings at the bottom of the hearing canal, the disorder of hearing function and a potential possibility of complications (paralysis of facial nerve, intracranial infections). Mycrobiologically the most present bacerium is Pseudomonas aeruginosa. Other microorganisms that can be isolated are Aspergillus species, Staphylococcus aureus, Proteus mirabilis, Klebsiella oxytoca, Candida parapsilosis. The disease is more common in patients with general diseases, mostly of metabolic nature (diabetes mellitus). The approach to diagnosis and therapy is usually urgent in order to prevent the development of serious complications. This paper shows the results of the analysis of clinical process of malignant otitis in 25 patients from the Clinic of ORL and MFS KCS according to the type of the cause and the development of potentially severe and life threatening complications.

SELF-EVÀLUATED HEALTH CONDITION AND LEADING HEALTH PROBLEMS OF THE REFUGEES FROM THE REFUGE CAMPS IN THE NORTH KOSOVO AND METOHIJA

Evaluation of population health condition is a basic mode of collecting health condition and health needs data of a population. Evaluation of population health condition is a base of contemporary health care and health service as well as health improvement and further health system development. In order to gain information on all (physical, psychological and social) health aspects, one has to go beyond health care system and consider objective health condition indicators gained by routine health statistics along with subjective information for creation an image of health condition, that is to say data collected from citizens themselves. This is significantly important when health condition data are sparse, as this is the case with health marginalized groups such as refugees and displaced persons. Value of data collected in this way is in the fact that they refer to determinants defining health, such as social-economical status, life style, education, employment, quality and availability of health care. The aims of this research is to establish how refugees, housed in refuge camps in the North Kosovo and Metohija, evaluate their health and what are the most common health problems of this population. The research was undertaken as a study of a standard representative sample of the adult refugees housed in the refuge camps in the North Kosovo and Metohija. It involved 109 surveyed people. The questionnaire, used for Health Examination in the Republic of Serbia adapted to local conditions, was utilized as a research instrument. The data were processed by descriptive statistics methods 2 and ÷ test was used in the means of data analyzing. More than a half of the surveyed (55.1%) answered that their health condition was good and very good. There is a great difference in health self-evaluating regarding age, marital status, education, the year when they arrived to current domicile. Less than two thirds (65.2%) of the surveyed in the refuge camps in North Kosovo and Metohija claimed that they suffer from some mentioned chronic illnesses.

CARDIAC DYSRRHYTHMIAS IN HYPERTENSIVE PATIENTS WITH LEFT VENTRICULE HYPERTROPHY

Cardiac dysrrhythmias can be found in patient with no signs of ischemic cardiac disease but supraventricular and ventricular arrhythmias can be developed in cardiac hypertrophy. In this work we have examined frequency of cardiac dysrrhythmias in patients with hypertension and with or without hypertrophy of left ventricule (LVH). Investigation was accomplished on 80 patients with high blood pressure. In all 80 patients, basic on ECHO signs we define LVH, and by using of Holter monitoring we analised dysrrhythmias. Basic on ECHO signs we have 2 groups of patients: I.group ECHO (+) LV, and II group ECHO (-) LVH, in each 40 patients. Holter monitoring have 37 patients [22 mans (59,45%) and 15 women (40,54%)]. Hypertensive patients with LVH, who hade dysrrhythmia, have middle value IMLV 155,3 ± 27,21; аgе 56,42 ± 6,04; duration of hypertension 11,73 ±10,05; and EF% 55,2 ± 3,77. In patients with Holter monitoring 16 ( 43,32%) have dysrrhythmias type II, IIIa, IIIb and IV Lowny degree, (12) 34,43% from 43,32% have LVH. Salves VES are registrated only in group hypertension patients with ECHO(+) 5,4%. By using Mann Whitney statistic test for presents of dysrrhythmias in 2 groups of patients (ECHO LVH and ECHO without LVH), we found statistics significants (p = 0,049). In IMLV 60-110 g/m² 8,82% patients have dysrrhythmia, IMLV 151-200 g/m² dysrrhythmias are registrated in 40%; and 50% patients have dysrrhythmias in IMLV> 200g/m². In group hypertensive patients with LVH dysrrhythmias are registrated in group with ecscentric LVH 35,1%

AEROBIC PARAMETERS OF ELITE SPORTSMAN

Parameters of anaerobic capacity were investigated in the group of 90 young subjects classed into diferent groups depent on their physical fitness. The group consisted of 30 wrestlers, 30 rowers and 30 students. Basic antropometric parameters were taken for each person and than Wingate anaerobic test was administrated. The aim of this investigation was to measure anaerobic parameters and determine weather there was a difference among sportsman and non sportsman. All parameters of Wingate test (anaerobic power, explosive power, anaerobic capacity) were significantly higher in the group of sportsman. (p0.05) Also, the highes values od anaerobic parameters were detected in the group of wrestlers, since this is typicaly anaerobic sport. The lower values were measured in the group of non sportsman, as expected.
