EARLY DIAGNOSIS OF MALIGNANT MELANOMA


Cutaneus melanoma is the most malignant tumor of the skin. Melanoma arises from the malignant transformation of melanocytes at the dermal-epidermal junction or from the nevomelanocytes of atypical melanocytic nevi, that become invasive and metastasize after various time intervals. Dermoscopy is a noninvasive diagnostic technique in dermato- venereology that includes the inspection of the lesion with a handheld dermoscope. Dermoscopy enables the evaluation of the specific morphological structures of the skin that are not visible to the naked eye.therefore, it links clinical dermatology and dermatopathology. The major goal is to differentiate pigmented lesions into melanocytic and nonmelanocytic lesions, and make further differentiations within each group. Furthermore, it is essential to distinguish melanoma from other me- lanocytic and nonmelanocytic skin lesions, and to diagnose melanoma and amelanotic melanoma in their early stages. Der- moscopic follow up includes long-term and short-term monitoring, and is an essential diagnostic technique in the manage- ment of pigmented skin lesions.

THE EFFECT OF CHANGE OF LIFESTYLE AND THERAPY OF METFORMIN IN NEWLY DIAGNOSED PATIENTS WITH TYPE 2 DIABETES

Metformin is an anti-hyperglycemic agent used for the treatment of type 2 diabetes mellitus. Biguanides lower blood glucose levels primarily by decreasing the amount of glucose produced by the liver. Metformin also helps to lower blood glucose levels by making muscle tissue more sensitive to insulin so glucose can be absorbe. The purpose of the study was to establish the effect of metformin on insulin secretion in obese patients with newly diagnosed diabetes mellitus type 2. We included 40 patients with Type-2 Diabetes Mellitus, who were treated on Department for Internal Disease in Hospital King Milutin Laplje Selo. Patients included in the study were newly diagnosed with diabetes mellitus type 2, glycemia < 12 mmol/L, and hed the Body Mass Index >30 kg/m2. They were treated with metformin, 1000 mg/day. Insulin sensitivity was determined by HOMA IR, insulin secretion by HOMA, lipid status by spectrophotometry. After six months of treatment blood biochemistry tested again. BMI was also established. Metformin significantly lowered BMI (BMI before therapy 31.02 kg/m2, after treatment 28.7 kg/m2) (both p<0,001 vs baseline) waist circumference mean value 98±0.55 cm-93,12 cm. (p<0,001) During the therapy the glycaemia decreased from 11.7 mmol - 6,78 mmol/l, insulinemia mean value 20.141 mlU/l -13.691, HOMA IR elevated in all patients. Among the cholesterol fractions, most significant results were obtained in LDL cholesterol. Mean LDL cholesterol levels 4.51± 2.02 mmol/L. Mean cholesterol level 6.98±2.31 mmol/L. HDL levels -1.32 mmol/L. Serum triglycerides was (mean) 2.63±1.32mmol/L. After six months therapy Metformin produces beneficial changes in lipid states. Metformin may be the first therapeutic option in the diabetes mellitus type 2 with overweight or obesity. Metformin produces beneficial changes in glycemia control, and moderated in weight, lipids and insulinemia.

FREQUENCY AND COMPLICATIONS ANGIOEDEMA QUINCKE AMONG HOSPITALIZED PATIENTS

Introduction: Approximately 15-23% of the general population have had urticarial rush during their lives. Goal: Examination of the urticarial frequency and urticarial etiopathogenesis among hospitalized patients. Sources and methodology: Retrospective analysis of the data extracted from patients' medical history at the Clinical Center of Kragujevac, Department for Dermatology and Venereology. Statistical methods used include proportional relations with tabular and graphical representation of the results gathered. Results: Out of 228 hospitalized patients in 2007, total of 28 (or 12.28%) were treated for Urticaria. The disease is more frequent among the following groups of patients: female population (total of 23, or 82.1%); people between 30 and 60 years of age (total of 15, or 53.6%); urban area population (total of 24, or 85.7%); patients with Dg. Urticaria acuta diagnosis (total of 19, or 67.8%); Dg. Oedema Quincke (total of 6, or 21, 4%); et people with cardiovascular diseases (total of 9, or 32.2%). The disease is more frequent in Spring, during the period April-June (11 patients, or 39.3%) with the greatest frequency in May (7 patients, or 25%). According to etiological samples the most prominent types of Urticaria among patients were: idiopathic (13 patients, or 46.4%), due to medications (5 patients, or 17.9%), nutritional origin (4 patients, or 14.3%), psychological factors (3 patients, or 10.7%). Considering complications, 6 patients (21.4%) had Quincke's Oedema, while 7 patients (25%) had allergic reactions to penicillin. Regarding outcome of the disease, total of 23 patients (82.1%) have been successfully cured. Discussion et Conclusion: The greatest number of serious urticarias with complications has been inducted by non-physical factors, mostly of immunological nature, while physically inducted urticaria have had less severe symptoms and required shorter time to cure. Nevertheless, exact causes for most types of urticaria are unknown, so continual research of causes, as well as courses, complications and treatments of this serious disease is necessary.

TRICHOPHYTID REACTION TO ANTIGEN AGENT IN CHILDRENS AGE

The main goal of this research was to present the mikid generalised reaction in children who suffered from trichophytiae profundae capillitii. After the infection and the inflammation brought on by the direct contact with the sick calves, the intensive mikid reactions occured, that is, the aseptic lesions of the skin. From 149 sick children, hospitalised at the DVC in Priština, 63(42,28%) were with the diagnosis of Kerion celsi. In 7(11,11%) cases trichophytid reactions occured to the animal type of the trichophyton mentagrophytes. The inflammed regions of the kerion produced the secondary bacterial infection with the regionale lymphadenopathy. Two weeks later antibodies and skin rash were produced in response to the infection agents. Early diagnostice and the adequate therapy decreases frequency of the mikid reaction in childrens age

PEMPHIGUS VULGARIS - THE CLINICAL MANIFESTATIONS AND COMPLICATIONS - case report

In this work the authors have treated patient with a wide range of skin symptomes and a various clinical sings and
manifestations of this serious bullous skin disease. The complications such as secondary bacterial infections on eroded
areas and swallow difficulties, because mucous membrane of oral cavity was affected, have supplemantary disabled quick
and efficient therapy. Therapeutical problems were presented as weakness, exhaustion, obstruction syndrome and asthmatic
constitution of a patient. The high doses of corticosteroids could provoke a hypertension, but still, with gradually reduced
doses of corticosteroids, supported by local therapy with antibiotics and corticosteroids, we achieved rehabilitation and
healing of our patient

VITILIGO - case report

Vitiligo manifests with hypopigmentated and depigmentated plates on the skin and mucous membranes. Etiopathogenic mechanism of disease appearing is not completely explained, though there are theories explaning the appearance of the disease. Precipitant factors, especially psyhotic stress, are very important for the appearance of the disease. The
patients usually come because of cosmetics defects, because vitiligo areas appear mostly on uncovered parts of a body. We
are presenting a girl, with the first sings of the disease appeared when she was seven, located symetrically periorbitally and
in capilicium followed by polyosis. The beginning of the disease might be connected with a psychotic stress.
