Chemical risk factors responsible for the formation of wedge-shaped lesions

Introduction: Non-carious tooth substances loss pose a major health problem of a modern man. The literature often collectively describes all non-carious lesions and is therefore difficult to compare results obtained by different authors. Chemical factors are one of the predisposing factors responsible for the formation of wedge-shaped erosions. Aim: Examination of chemical risk factors as one of the predisposing causes responsible for the formation of wedge-shaped lesions. Method: We examined 62 patients with wedge-shaped erosions (mean age 45.52 ± 12.03 years, 58.1% of men) and 60 patients without erosions in the control group (mean age 34.40 ± 9.28 years, 60% men) . The entire examination was completed by using a questionnaire at the Dental Clinic of the University of Pristina - Kosovska Mitrovica. salivary pH was measured by the pH meter. Results: The results show that the wedge-shaped lesions often occur equally in both men and women. Considerably often it might appear in older people but can also occur in teenagers. Patients with wedge-shaped erosion have increased acidity of saliva, a heightened sense of acid in the mouth and consume a lot more carbonated drinks compared to patients without erosions. Conclusion: Wedge-shaped lesions are more common in people older than 40 years. Taking into account the results obtained in this study it can be concluded that the chemical risk factors truly fall within the predisposing factors that may be responsible for the creation of wedge-shaped erosions.

MALIGNI TUMORI VELIKIH PLJUVAČNIH ŽLEZDA MEĐU STANOVNIŠTVOM GRADA BEOGRADA

Maligni tumori pljuvačnih žlezda su retka oboljenja i u najvećem broju se javljaju u velikim pljuvačnim žlezdama. Cilj istraživanja bio je da se analizira trend incidencije malignih tumora velikih pljuvačnih žlezda kod stanovništva grada Beograda. Podaci su dobijeni iz Registra za maligne tumore centralne Srbije. Analizirani su svi slučajevi malignih tumora velikih pljuvačnih žlezda registrovani u Beogradu od 01. januara 1999. godine do 31. decembra 2010. godine. Trend kretanja i godišnji procenat promene incidencije izračunat je regresionom analizom uz pomoć tačaka spajanja (engl. joinpoint regression analysis). U ispitivanom periodu Registru je prijavljeno je 283 pacijenta (168 muškaraca i 115 žena) sa malignim tumorima velikih pljuvačnih žlezda. Standardizovana stopa incidencije svih tumora u ispitivanom periodu, za oba pola, bila je 0,87/100,000 stanovnika, odnosno 1,13 za muškarace i 0,67 za žene. Standardizovana stopa incidence se uvećavala sa godišnjim procentom promene od 6,4% kod oba pola (p<0,001). Rezultati su pokazali konstantno povećanje trenda incidencije malignih tumora velikih pljuvačnih žlezda kod stanovništva grada Beogradu između 1999. i 2010. godine.

Promene na pulpi i dentinu nakon direktnog i indirektnog prekrivanja alkalnim cementom

Suštinu biološkog lečenja caries profunda čini proces 'ozdravljenja' koji se zasniva na stimulaciji dentinogenetske funkcije pulpe, što omogućava stvaranje mineralizovanog tkiva. U eksperimentalnom istraživanju postavljen je cilj da se skening elektronskom mikroskopijom i polarizacionom mikroskopijom: Analiziraju promene na celularnim i ekstracelularnim komponentama zubne pulpe posle direktnog i indirektnog prekrivanja alkalnim cementom (Dycal), Utvrdi izgled dentinske površine posle direktnog i indirektnog prekrivanja pulpe i Formira predlog za kliničku primenu preparata Dycal ukoliko obezbeđuje dobru i efikasnu dentinogenezu. Ispitivanje je izvršeno na eksperimentalnim životinjama (domaća svinja). Za ovo istraživanje korišćen je alkalni cement Dycal. Na zubima eksperimentalne grupe uradjene su preparacije V klase sa vestibularne strane. Na jedanaest zuba je uradjeno indirektno, a na jedanaest zuba direktno prekrivanje pulpe. Nakon završene preparacije dentin je direktno prekriven alkalnim cementom Dycal. Preko Dycal-a je postavljena podloga cink fosfatnog cementa i materijal za definitivno zatvaranje kaviteta (amalgam). Na zubima gde je urađena perforacija komore pulpe Dycal je ubacivan direktno u komoru pulpe, a preko toga je postavljena podloga cink fosfatnog cementa i kavitet je definitivno zatvaran amalgamom. Adekvatno pripremljeni zubi i pulpa posmatrani su SEM i polarizacionim mikroskopom. Rezultati: Posmatranjem i analizom dobijenih rezultata, na polarizacionom i skening elektronskom mikroskopu u poređenju sa kontrolnom grupom, dobijeni su rezultati koji mogu imati značajnu kliničku implikaciju u biološkom lečenju pulpe. Dycal direktno apliciran u komoru pulpe: uočava se granica između novostvorenog reparativnog dentina u normalnu dentinsku strukturu, sreću se degradirane smežurane ćelije, zapažaju se jezgra kalcifikacije, mladi krvni sudovi i ćelije (fibrociti, histiociti i fibroblasti). Indirektno aplikovan Dycal: uočava se normalna dentinska struktura , jasno su vidljivi dentinski kanalići, a sreće se i atubulusna dentinska struktura, vidi se slika dentinske strukture koja prožima acelularno pulpno tkivo, hipercelularnost na granici stvaranja novog dentina i brojni krvni sudovi. Prisustvo amorfnog kalcifikata dentinske strukture govori o početnom stvaranju barijere od čvrstog tkiva što je cilj eksperimenta. Dobijeni rezultati eksperimentalnim istraživanjem sugerišu Dycal kao dobar materijal za direktno i indirektno prekrivanje pulpe.

CONSERVATIVE CARE OF ENDODONTIC TREATED TEETH WITH GREAT DAMAGE OF CROWN- REVIEW OF FOUR PRACTIC CASE

There are different stick types and shapes, different root canal preparation techniques, as well as different materials for stick cementing and making annexes around posts. The use of fiber-reinforced resin posts to restore endodontically treated teeth has gained popularity due to its mechanical and esthetic characteristics as well as properties of modern adhesive systems. The aim of this article was to show a clinical technique to reconstruct endodontically treated teeth with great loss of tooth structure using direct fiber-reinforced post systems and direct composite restorations. After the preparation of the root canal and cementation of a FRR post, particular attention was paid to the incremental and curing techniques adopted to reconstruct coronal part of the tooth. It is possible to place of fiber-reinforced resin posts.

VITAL PROCEDURE ON MULTIROOT TEETH OF LOWER JAW

Vital extirpation is microsurgical intervention.Wellknowing of internal morpfology of pulp cavity and with patience, carefull and precise work is possible to extirpate, dilatate and fil in both distal and mesial root of down molars,even if in mesial root exists two canal (buccal and lingual) what is the ruosh aften finding. Vital extirpation is very frequenthy used method in the treatment of multi-root teeth.It is more requested intervention tlak usnal once,but patients mane less visits at the dentist and the results one good enongh. So, one session method isacceptable as from medical point of view but endodontic too.

ENCE ON INADEQUATE DENTAL RESTORATION IN STATE OF PARODONTIUM INFLUEN

After restoration of approximative cavities where gingive wall is forming subgingivaly occurs an damage of epitel of gingival sulcus and subepitel fibrous tissue. These damages occur partially in order of direct irritative effect of materials, and partially because of inadequately made definite reisen. Anomaly which may occur while making reisen, is effect of micro leakage between reisen wall of cavities roughness and reisen parts which prominate above anatomic tooth sphere , enable faster and bigger accumulating of dental plaque. Gingive inflammation is widening deeper in parodontium trough vascular network situated in interdental bone septum. Inflammation is constantly spreading from the point and reaches parodontium. The aim of this study is to estimate clinical changes in parodontium within certain group of patients of different age and sex, caused by inadequate dental restoration. Results of this research present that anomaly in dental reisen , have great influence on inflammation reaction of parodontium. Composite reisen cause higher degree of inflammatory reaction in compared to amalgam reisen

MAST CELLS IN PERIODONT PERIODONTAL DISEASE DISEASE

Periodontal disease is a chronic inflammatory disease that is largely attributable to infections with gram-negative bacteria and is characterised by both gingival inflammation and alveolar bone resorption. Beside macrophages, the second most common cells of the gingival inflammatory infiltrat are mast cells. Mast cells are heterogenous cell population which live from six months to one year. These cells are activated by differnt immunologic and non-immunologic signals. Activated mast cells can secrete a range of substances that regulate angiogenesis, tissue remodeling and wound healing, which include both degradation and synthesis of tissue components. Apart from that, mast cells have an important role in phagocytosis, antigen processing and presentation to T-cells. It is confirmed that gingival mast cells can express matrix metalloproteinases (MMPs) and the tissue inhibitors of metalloproteinases, TIMP-1 and TIMP-2. This indicates that mast cells are likely to play an important role in inflammation and tissue degradation in periodontal diseases.
