Operative treatment of supracondylar elbow fracture in a child using the percutaneous method

Supracondylar fractures are the most common elbow injuries in children and are associated with prolonged morbidity due to possible complications that can lead to deformity. The decision on the treatment method is made based on Gartland's classification (I, II, III and IV types) and the treatment can be non-operative (I and II type) and operative (III and IV type). When it comes to the percutaneous method, the main dilemma for its implementation is related to pinning from the medial side of the elbow because there is a high possibility of injury to the n. ulnaris which, according to data from the literature, occurs in some 15% of cases. The aim of treatment is pain relief and maintenance of the patient's functional status. The case presented in this paper represents a patient with whom the clinician is most likely to encounter and shows the clinical assessment of the patient's condition, the way of deciding on the treatment method and the outcome of the treatment undertaken. Agirl, 8 years old, was injured when she fell while playing. At the Department of Orthopedic Surgery and Traumatology, Clinical Hospital Center Kosovska Mitrovica, the patient was clinically and radiographically examined, and the injury was defined as a supracondylar fracture type III according to Gartland. After adequate preoperative preparation under general anesthesia, without the use of a drape - Turniquet, with the use of a C-bow, repositioning is performed and after obtaining a satisfactory position of the fragments, they are fixed percutaneously with 3 Kirschner needles, two medially and one laterally. The patient was discharged 3 days after admission with controls performed for 7 days. The Kirschner pins were removed on the 5th week after the operation and physical treatment was started, after which the movements of flexion and extension as well as pronation and supination were fully restored. Similar results are found in the literature. This information can be helpful in advising parents about what to expect after their child's injury. Also, they represent evidence of good clinical practice for orthopedic doctors and physiotherapists.

Treatment fracture of the diaphisis humerus with functional plaster

Treatment of humerus fractures is divided into operative and non-operative treatment Fractures of the diaphysis of the humerus heal well. Surgeons today have many opportunities to treat them. The decision on the type of treatment to be applied depends on the location of the fracture, the existence of associated injuries, the age and the general condition of the patient. Non-operative treatment is most often applied, although there are fractures in which surgical intervention is necessary in order to perform healing and prevent complications. Non-operative treatment of fractures of the diaphysis of the humerus gives good results, with little angulation and minimal or no shortening of the arm. Adequate repositioning, appropriate plaster immobilization and regular X-rays heal the fracture within the allotted time. Disciplined early physical therapy in terms of circular movements prevents shoulder contracture and allows later physical therapy to last significantly shorter. Non-operative treatment lasts from 7-11,5 weeks.

Analysis of premature births for the period from 2009. to 2013. in Health Center Kosovska Mitrovica

Delivery before 37th week of gestation is defined as preterm, independently of body mass of babies, according to the World Health Organization. Premature birth is the current problem in the world due to the high risk of neonatal morbidity and mortality and incompletely clear etiology. In our country the frequency of preterm delivery is 6%. We have retrospectively analyzed early deliveries in Health Center Kosovska Mitrovica in the period from 2009 to 2013. In this five-year period, there were totaly 3398 deliveries, of which 148 or 4.35 % were preterm delivery. The aim of this study was to investigate the incidence of premature birth in the five-year period in relation to: the total number of births in a given period, the age of the patients, the manner of completion of delivery, gestational week of pregnancy, parity and the most common causes that led to the PTP. In the analysis we have included the body weight and Apgar score of premature infants. We have used protocols of births, maternal history of disease, neonatal protocols and lists of newborns. Most of them were multiparas (41.2 %),56.7 % of pregnant women were aged of 21-30 years and pregnancy in 79.9 % of cases ended with 33 to 37 ng. Vaginal deliveries were completed in 84.4% of premature births, and caesarean in 15.6%. Twin pregnancies with preterm deliveries were represented in 10.2%. The most common weight of premature infants ranged from 2000 to 2500 grams (48.6 %). The most common causes that led to the PTP were PPROM (22.9 %), unknown causes (27. 1 %), multiple pregnancy (18.2 %), genital infection (8.7 %), cervical incompetence (6.7 %), IUGR (5.4%), PIH (4.2%), placental abruption and placenta previa (2.1 %) and other (4.7%) .

COMPRESSIVE OSTEOSYNTHESIS AND BONE OSTEOPLASTICS AS METHODS IN TREATMENT OF BONES PSEUDOARTHROSIS

The pseudoarthrosis is a pathological state of the bone when the refracted bone fragments are not connected by bone callus. The causes for the occurrence of the pseudoarthrosis may be general and lokal.In treatment we were using two methods: the bone plastic and osteosynthesis and compression osteosynthesis by Ilizarov. The aim of this work is the analysis of patients with pseudoarthrosis and results of treatment. The study included 29 patients treated for the past ten years the Department of Orthopedics Health Center Z.C.Kosovska Mitrovica.The most frequently pseudoarthrosis were in humerusu 6 (21%), ulna 6 (21%) and skafoidne bone 6 (21%). The pseudoarthrosis in tibia was treated in 4 (14%) patients, in the femur 3 (10%) patients. 2 (7%) of the patients were operated with diagnosis the medial-maleolus pseudoarthrosis of the tibia fractures and 1 (3%) patient were operated with diagnosis the maleolus pseudoarthrosis of the fibula and of the radius.We were using the treatment methods osteoplastic and osteosynthesis for 28 (97%) patients and one patient was treated with the device by Ilizarov method. Patients were monitored by the clinical way, by the radiographic way ,by laboratory way and by the functional way The average time of the monitoring was ten months .The average time of the healing was the five months. We noticed the one complications only, a lesion of the radius, which is repaired. The pseudoarthrosis is not common, in our material it is 4% of the total number of the operated patients.The most frequently pseudoarthrosis was on humerous 21% (6 patients). By applying the compressive osteosynthesis methods and osteoplastic method we have achieved the completely curring in 89% (26 operated patients).
