The impact of neuropsychological assessment in epileptic focus localization

Introduction: Epilepsy that takes place in the temporal lobe is manifested through three symptoms: aura, temporal pseudoabsence and automatisms, while right-sided temporal lobe epilepsy had been also linked with visual or image learning and memory deficits. Case outline: Patient 24 years old in 2020. In July 2020 had complex partial seizures with an unordinary psychomotoric reactions and verbalization, followed by qualitative conscience change in the form of confusion and amnesia. Therapy with paroxetine showed no result. Neuropsychological evaluation showed no other cognitive deficit than severe visuoconstruction dyspraxia and discrete decrease in complex attention. In behavioral aspect prodromal symptoms of tension, anxiety and depression were occurred. Conclusion: Neuropsychological assessment is, along the magnetic resonance imaging and electroencephalographic, the most important method for indicating cognitive deficit in epilepsy and determining the epileptic focus.

VESTIBULAR NERVE SCHWANNOMA, TREATED WITH GAMMA KNIFE AND DIAGNOSED AFTER TRANSITORY ISCHEMIC ATTACK IN POSTERIOR CEREBRAL VASCULARISATION

A case of a patient is shown, who was admitted for neurological exploration, with clinical manifestations of the cerebrovascular accident in the posterior circulation (TIA V-B vascularisation). In accordance with diagnostic protocol, an endocranial CT was performed. The results of the brain CT indicated an existence of an alteration in the right pontocerebellar angle area, which was subsequently diagnosed as a n. vestibularis schwannoma in an NMR examination. After a neurosurgical deliberation, the patient was treated with a gamma knife. After this surgery, which was done in a local anesthesia, the patient's condition was well and he was discharged. Control examinations have shown minor difficulties during the stimulation of the left ear auditive structures. Control examinations involving NMR showed no signs of recidivate focal findings.

Cognitive functions of patients with epilepsy treated by topiramate

Topiramate is a newer drug with a strong antiepileptic potential, but also with adverse effects, especially on cognitive functions. This review deals with the study covering the impact of topiramate on cognition in patients with epilepsy. The most common side effects are included somnolence, psychomotor slowing, difficulty in memory, concentration and attention disorder and problems with speaking. Most studies have shown that a high initial dose, rapid titration and application in polytherapy increase the risk of cognitive impairment after-introduction of topiramate. In addition, depression and early onset of the disease can also contribute to cognitive adverse effects, even at low doses of the drug. Although topiramate indisputably linked to the risk of interference cognitive impairments, some studies suggest their reversibility. This, together with high efficiency antiepileptic obliges researchers to continue searching for the risk factors and mechanisms of cognitive side effects in order to define the profile of patients who would be of maximum benefit of the drug in controlling seizures with absent or acceptable side effects.
